Little is known about racial disparities in the quality of end-stage renal disease (ESRD) care in the United States. We used data from the Medicare 5% claims database to examine race/ethnicity-based disparities in the quality of ESRD care among Medicare patients, using a pay-for-performance quality measure. Patients who initiated dialysis from January 1, 2005 to December 31, 2006 and survived to December 31, 2006 were identified from the National Medicare ESRD Program (NMP) dataset. We assessed the relationship between race/ethnicity and the observed-to-expected (O/E) ratio of this quality measure by using a logistic regression model. The adjusted mean O/E ratio was significantly lower among Black, Hispanic, Asian, and American Indian patients than among non-Hispanic White patients (1.23, 1.13, 1.16, and 1.14 vs. 1.45, respectively). Among patients with diabetes, Blacks, Hispanics, and American Indians experienced significantly lower O/E ratios of the quality measure than did non-Hispanic Whites (0.87, 0.85, 0.86, and 0.88 vs. 1.13, respectively). The lowest quality was observed among patients who received transplant and were black, Hispanic, and American Indian. Compared with Whites, other ethnic minorities have less quality of care for ESRD patients in the United States. Disparities in care quality persist even among patients who receive care from a transplant center.
